Once you have scheduled your evaluation, you will need to submit your MMJ patient form and state ID. The patient form can be downloaded from the Virginia Marijuana Card website. Connect with a Medical Marijuana Doctor

After you have submitted your patient form and state ID, you will connect with a licensed medical marijuana doctor online using your phone or computer. The doctor will review your medical history and ask questions about your condition. If you qualify for a medical marijuana card, the doctor will issue a written certification that you can use to apply for your medical marijuana card.

Receive Your Medical Marijuana Card
